About the Role:
Join PACIFIC HEALTH SERVICES INC as a Home Health RN and make a meaningful impact in the lives of patients in Apple Valley, CA. We are looking for compassionate and skilled nurses to deliver exceptional care in a home setting, ensuring comfort and support for our clients and their families.
Day Shift: Mon-Friday 8am-12pm
Responsibilities:
- Conduct comprehensive assessments of patients' health and care needs.
- Develop and implement personalized care plans in collaboration with healthcare teams.
- Administer medications and treatments as prescribed by physicians.
- Monitor and document patient progress and respond to changes in condition.
- Educate patients and families on health management and home care techniques.
- Coordinate with interdisciplinary teams to ensure holistic patient care.
- Maintain accurate medical records and adhere to compliance regulations.
- Provide emotional support and guidance to patients and their families.
Requirements:
- Valid RN license in the state of California.
- Minimum of 1-2 years of nursing experience, preferably in home health or community settings.
- Strong assessment, critical thinking, and communication skills.
- Ability to work independently and manage time effectively.
- Current CPR and First Aid certification.
- Compassionate demeanor with a patient-centered approach to care.
- Proficient in electronic medical records (EMR) systems.
- Willingness to travel to patients' homes within the service area.
